Final determination for claims payments rests with the insurance
company and will be based on the group insurance contract in effect at
the time the claim is incurred.
The benefit and rate information is meant to be accurate and
timely. However, your plan and costs are subject to change
periodically, usually once a year at the plan’s anniversary/renewal
date. Benefit reviews are usually done one to two months prior to the
renewal date, and new information posted and distributed approximately
two weeks prior to the renewal date. |
